The social work/case management staff work with each resident to ease the transition into our facility, whether for short-term or long-term care. Our goal is to provide a supportive “family” environment and help each resident achieve his/her rehabilitative goals and desires.
We assist the resident and family from admission through the discharge process. Our objective is to support the emotional, social and financial concerns so that the individual can focus on their rehabilitative recovery. Our staff of knowledgeable clinicians will assist you with identifying services needed and coordinating these community services so that a safe and timely discharge can occur. For individuals admitted for long-term residency, our social work staff will assist you to achieve a meaningful routine based on your desires and preferences.
Many of our residents require special diets, and that is why members of our clinical dietary team, including our dietitian, meet with the resident to review medical considerations that will require specific meal preparations in accordance with resident desires and physician recommendations. Our speech and language pathologist and occupational therapist will evaluate each resident upon admission to ensure that each person receives the proper assistance with meals, including staff support and equipment such as special eating utensils.
Throughout the year special menus are planned in response to the seasons and holidays, or just special events such as our summer BBQs. We will use any excuse for a celebration, and all celebrations require delicious food!
Throughout the year there are many special events that family members and visitors are encouraged to attend. We also recognize that visits from family members are important. To allow for maximum visiting time, we will provide a meal to the visitor with our resident meal. That way you can share a meal together and extend the visit.
